Patient Forms

Registration paperwork will be provided at the office as well. This paperwork is also provided online if patients are able to print and complete the paperwork prior to their appointment.

Release of information must be completed for communications between our office and anyone else. This form is included in the registration paperwork but also added independently here if needed. Referring providers are often added to release of information forms but any other individual the patient wants to include may be added.

These privacy policies are made available to you in the office but also may be found here. Please contact the main office phone number if there are any questions.

Payment Options

You can pay co-pays or private pay rates here. Unless our providers are out of network with your insurance, the patient's primary insurance will be billed first, followed by secondary insurance if applicable. Any remaining balance will be sent in a statement to the patient. The patient is responsible for understanding if there is a deductible associated with their insurance, which may result in a higher patient payment responsibility. You may contact our office prior to your appointment to inquire about the CPT codes used to bill insurance in order to verify what may be covered with your insurance.

Pay the amount specified on your billing statement by clicking the "Pay Bill" button. Payments are processed through Square. If you would prefer to make a partial payment, you will need to contact our office to authorize a payment plan. Billing statements are sent to you in the mail. If you prefer not to pay online, checks are also accepted and may be sent in the mail.
